What’s your clinic’s first impression on patients? When patients enter your clinic, what they see and feel is critical to creating a lasting relationship. These 60 second first impressions can set the stage for the overall experience. Worn upholstery, dirty carpeting, chipped walls, a messy front desk - all normal wear and tear, but what does the patient feel when they experience these images? The messages sent to the mind are very subtle; they’re related to feelings which can have positive or negative overtones. General Rule: If it wasn’t documented, it wasn’t done. History Tip: For the initial hospital care codes (99221-99223), the two highest levels require: - A comprehensive history:
- Extended history of present illness – 4 elements
- Complete review of systems -10 systems
- All three past, family and social history elements
- A comprehensive exam
- 8 systems for a general multi-system exam
- Either moderate complexity medical decision making (99222) or high complexity medical decision making (99223)
